REGISTRATION FORM INSTRUCTIONS
SECTION A. PERSONAL INFORMATION
Complete information. All participants must be registered, and badges must be worn to gain entrance into all sessions and events.
SECTION B. FULL OR ONE-DAY REGISTRATION
Full Registration—Fee includes admission to educational sessions, Presidential Plenary, exhibit hall, and one copy of meeting materials. Residents and full-time students must mail or fax a photocopy of their student identification card when submitting the registration form to be granted the resident/student rate.
One-Day Registration—Fee includes admission to educational sessions, exhibit hall, and AAGP social functions on the day registered. Also includes one copy of meeting materials.
SECTION C. INTENSIVE WORKSHOPS
Space may be limited for these programs, so please register early. Register for only one, as they are concurrent sessions.
SECTION D. GUEST TICKETS
Spouse/Partner Registration $175
Admission to the Presidential Plenary, Exhibit Hall, and all non-ISS symposia.
Awards & Networking Reception $65 Adult / $20 Child (under 13)
Admission to the Awards & Networking Receptions is included in the Full and Spouse/Partner Registration fee. Additional tickets may be purchased for $65 per adult/$20 per child. The number of tickets available for purchase on-site is limited.
Exhibit Hall Only $40
Admission to the Exhibit Hall is included in the Full, One-day, and Spouse/Partner Registration fee. Tickets are available to those accompanying a Full- or One-Day registrant. No ticket is required for anyone under 18 years of age. A waiver must be signed by a parent or guardian for those under 18 years old; the form will be available on-site.
SECTION E. GMHF ACTIVITIES
Contribute to the Geriatric Mental Health Foundation, AAGP's consumer-education arm, while registering for the meeting. All donations are tax deductible to the extent allowed by law.
Attend the GMHF St. Patrick's Day Party:
$75 Adult / $30 Child (4–12 years old)
The number of tickets available for purchase on-site may be limited.
SECTION F. AAGP-PAC ACTIVITIES
Support AAGP's political action committee (AAGP-PAC) by attending a southern-style buffet dinner on board the Georgia Queen Riverboat. Individuals may make voluntary contributions to AAGP-PAC using personal checks, made payable to AAGP-PAC, or with an individual Visa, MasterCard, or American Express credit card. Contributions from corporations and associations are illegal and will be returned. Under Federal law, only U.S. citizens or permanent resident aliens (green card holders residing in the U.S.) may contribute to, or be solicited for, a political action committee or a political campaign. Gifts to political action committees are not tax-deductible.
SECTION G. MEMBERSHIP FEES: JOIN NOW AND $AVE!
NEW Members: Become a NEW Full Member or AAGP Affiliate while registering for the 2010 Annual Meeting and save most of the cost of membership by qualifying for the lower member registration rate.
SECTION H. 2010 Annual Meeting Recordings
Sessions from the 2010 Annual Meeting will be available as synchronized digital audio/PowerPoint presentations. Hear sessions you weren't able to attend, print handouts, make session notes, and even download presentations to your MP3 player for a totally portable learning experience. Best of all, you can earn AMA PRA Category 1 Credits™.
SECTION I. METHOD OF PAYMENT
Registration payment may be made by credit card (Visa, MasterCard, or American Express) or check drawn on a U.S. bank and made payable to AAGP. All payments must be made in U.S. dollars. AAGP cannot assume fees for foreign bank transfers. Forms received without payment will not be processed and the registrant WILL NOT be registered for the conference. Please write legibly—failure to do so may result in delays.
CANCELLATION POLICY
A letter requesting a refund should state the registrant's name, address and to whom the refund should be made payable. The registration fee, minus a $75 processing fee, is refundable if written notice is received by AAGP on or before February 11, 2010. After this date, the registration fee is not refundable. Refunds will be issued 6–8 weeks after the meeting. AAGP is not responsible for any charges or cancellation fees assessed by airlines or hotels.
AMERICANS WITH DISABILITIES ACT
AAGP wishes to take any steps required to ensure that no individual with a disability is excluded, denied services, segregated, or otherwise treated differently due to the absence of auxiliary aids and services identified in the Americans with Disabilities Act.
